UAB Medicine in Birmingham, AL is seeking a Registered Nurse for Case Management on a PRN days schedule. The role coordinates the interdisciplinary plan of care from admission to discharge and collaborates with patients, families, and the care team to ensure successful transitions.

The ideal candidate holds an Alabama RN license (or eligibility), a Diploma in Nursing, and a BLS certification, with at least one year of RN experience.

• BLS certification within 30 days of hire date.
• Alabama RN license or eligibility required.
• Diploma from an accredited nursing program and professional licensure at time of hire.

Job Description

Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.

• Creates plan for care across the continuum, integrating patient/family preferences and values.
• Monitors patient medical necessity and level of care through assessments, ongoing evaluations
  and/or patient records.
• Advocates for resources and removal of barriers.
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective
  implementation and reevaluation of health plan.
• Acts as a resource for adequate medical record documentation, level of care recommendations, and
  services as they relate to diagnoses, and treatment options for post-discharge care.

Qualifications

Licenses / Certifications / Registrations

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.

Education:

• Diploma from an accredited school/college of nursing and professional licensure at time of hire required.
• One Year of RN Experience Required
