Job summary

UAB Medicine is seeking a registered nurse to coordinate the interdisciplinary plan of care for patients from admission to discharge. The role is based at UAB St Vincent's Birmingham and features three 12-hour shifts, 7am–7:30pm, Saturday through Monday, with comprehensive benefits for eligible positions.

You will monitor care needs, advocate for resources, and collaborate with the care transition team to ensure smooth post-discharge planning and accurate documentation.

Qualifications

• BLS Provider within 30 days of hire date.
• Active Alabama RN license required.
• Diploma from an accredited nursing program.
• Ability to coordinate interdisciplinary care across admission to discharge.

• Creates plan for care across the continuum, integrating patient/family preferences and values.
• Monitors patient medical necessity and level of care through assessments, ongoing evaluations
  and/or patient records.
• Advocates for resources and removal of barriers.
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective
  implementation and reevaluation of health plan.
• Acts as a resource for adequate medical record documentation, level of care recommendations, and
  services as they relate to diagnoses, and treatment options for post-discharge care.

Qualifications

Licenses / Certifications / Registrations

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.

Education:

• Diploma from an accredited school/college of nursing and professional licensure at time of hire required.
