Job summary

UAB Medicine in Birmingham, AL, is seeking a Registered Nurse in Case Management to coordinate the interdisciplinary plan of care from admission to discharge.

You will develop a care plan across the continuum with input from patients and families, monitor medical necessity, and advocate for needed resources while addressing discharge barriers. UAB Medicine offers strong benefits and opportunities for growth.

Qualifications

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
• Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.

• Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.

• Maintains ongoing dialog with supervisor and care transition team members to ensure effective
  implementation and reevaluation of health plan.


• Acts as a resource for adequate medical record documentation, level of care recommendations, and
  services as they relate to diagnoses, and treatment options for post-discharge care.

Qualifications

Licenses / Certifications / Registrations

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.


• Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.

Education:
