Job summary

Haven Hospice in Saint Augustine, FL is seeking a Hospice Admissions RN to provide compassionate, patient-centered care at the right time in patients’ lives. The role involves home visits, coordinating start of services, and collaborating with a multidisciplinary team to ensure high-quality end-of-life care.

Applicants should be a graduate of an accredited nursing program with current licensure in Florida and possess at least one year of hospice nursing experience.

Qualifications

• Graduate of an accredited nursing school with current licensure in the operating state.
• One year of hospice nursing experience required; 3+ years preferred or similar settings considered.
• Must be a licensed driver with insured automobile and in good working order.

Responsibilities

• Visit patients in homes, hospitals, or nursing facilities referred for hospice care.
• Explain hospice services and discuss goals with patients and families; obtain consents and documents.
• Obtain medical history, current medications, and symptoms; perform clinical assessments for eligibility.
• Coordinate start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team; maintain accurate timely documentation.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
