Job summary

Confidential is seeking a dedicated Registered Nurse (RN) to deliver quality nursing care in our facility, focusing on resident-centered outcomes. The right candidate will be a compassionate team-player who collaborates with an interdisciplinary care team.

Responsibilities include input for discharge planning, development of individualized care plans, and ensuring effective care with available resources to support residents and families. CPR certification is required within 90 days of hire.

• Current RN license in state of practice required
• Must hold and maintain CPR certification; CPR certification to be obtained within 90 days of hire

• Identify needs and provide input for discharge planning and follow-up throughout length of stay.
• Assist with development of individualized plan of care in collaboration with resident/responsible party and interdisciplinary care team.
• Implement plan of care consistently, effectively and cost efficiently with focus on resident centered outcomes by utilizing all appropriate resources for resident, family or responsible party counseling and discharge planning.
• Work in collaboration with physician and other health care professionals by sharing information relevant to changing plan of care.
• Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices.

Qualifications:

• Currently Licensed Registered Nurse (RN) in state of practice requiredMust hold and maintain a current CPR certification. Licensed nursing staff are required to obtain their CPR certification within 90 days of hire.License in good standing with the state. Must have no finding in the registry concerning abuse, neglect, or misappropriation of resident property.
