Job summary

Phoenix Home Care & Hospice in Bolivar, MO is seeking an In-Home Registered Nurse or Licensed Practical Nurse to join our compassionate team. You will provide in-home care, conduct assessments, take vitals, manage medications, and support clients in their homes across Bolivar and surrounding areas.

We offer full-time and part-time roles with pay per visit ranging from $25 to $35, plus competitive benefits and training. Must hold MO RN or LPN license and have a valid driver’s license.

Qualifications

• Active MO RN or LPN license is required.
• Home care experience is preferred but not required.
• Strong organizational and self-management skills.
• Valid driver’s license is required.

Responsibilities

• Complete admission assessments and reassessments for in-home clients.
• Provide nurse visits for adults and seniors: vitals, med management, labs.
• May provide supervisory visits for caregiver services.
• Provide instruction/education on medications.

Responsibilities:

• Complete admission assessments and reassessments for In-Home caregiver population.
• Provide nurse visits for adult and senior population: Taking vitals, medication management, monthly diabetic nails, and labs.
• May provide supervisory visits for caregiver services
• Provide instruction/ education on medication
• Good time management, detail oriented and self-starter

Requirements:

• Active Registered Nurse or Licensed Practical Nurse license in the state of MO
• Home Care experience preferred but not required
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver’s license
