Job summary

Phoenix Home Care & Hospice in Missouri is seeking a compassionate RN or LPN to join our In-Home team, delivering high-quality nursing care in clients' homes across Bolivar and surrounding areas. You will perform admissions, vitals, medicine management, diabetic nail care, and labs, with opportunities for caregiver supervision and patient education.

Valid MO license and driver’s license required; home care experience preferred.

Qualifications

• Active RN or LPN license in MO.
• Home care experience preferred.
• Valid driver’s license.
• Strong organizational and customer service skills.

Responsibilities

• Complete admission assessments and reassessments for in-home patients.
• Provide nurse visits: vitals, medications, diabetic nail care, labs.
• May perform supervisory visits for caregivers.
• Educate patients on medications.
• Demonstrate strong time management and attention to detail.

Responsibilities:

• Complete admission assessments and reassessments for In-Home caregiver population.
• Provide nurse visits for adult and senior population: Taking vitals, medication management, monthly diabetic nails, and labs.
• May provide supervisory visits for caregiver services
• Provide instruction/ education on medication
• Good time management, detail oriented and self-starter

Requirements:

• Active Registered Nurse or Licensed Practical Nurse license in the state of MO
• Home Care experience preferred but not required
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver’s license
