Job summary

Bristol Hospice in Costa Mesa, CA seeks a compassionate Registered Nurse to perform comprehensive patient and family assessments and deliver skilled nursing care within hospice guidelines. You will develop and revise plans of care, coordinate with physicians, and support patient self-care with families and caregivers.

The role requires CA RN licensure, 1+ year acute care experience, willingness to travel, and a patient-centered approach to terminally ill individuals.

Qualifications

• Must have at least 1 year of acute care experience in an institutional setting.
• 1 year of home care experience preferred.
• Must have current licensure in the state (CA).
• Understand hospice philosophy and needs of the terminally ill.
• Be flexible in work hours and travel across Bristol Hospice Service Area.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness(es).
• Provide professional nursing care by utilizing all elements of the nursing process.
• Assess and evaluate patient’s status by writing and initiating plan of care; regularly re-evaluate needs.
• Initiate the plan of care and revise as patient status changes; communicate with physician.
• Teach patient and family self-care techniques and involve them in planning.
• Work in concert with the interdisciplinary group to coordinate care.
• Record pain/symptom management and changes/outcomes as appropriate.
