Job summary

Bristol Hospice is seeking an Admissions Registered Nurse to establish and maintain relationships with customers and referral sources within the community, thereby expanding access to Bristol Hospice services in the areas we serve.

The Admissions RN will work with the DPCS, ED and Community Education Director to promote hospice education, coordinate admissions, complete consults, and ensure timely admissions while traveling up to 20% as needed.

Qualifications

• Must have a current RN license in the state
• Minimum of 1 year of nursing experience in the last 3 years or BSN from NLN-accredited program
• Willingness to travel 20% and flexible hours
• Strong communication and organizational skills
• Understanding of hospice philosophy and patient-centered care
• Ability to work autonomously with minimal supervision

Responsibilities

• Establish and maintain relationships with customers and referral sources
• Promote hospice access and educate the community about Bristol Hospice services
• Coordinate admissions and referrals to ensure timely admission
• Complete hospice consults/evaluations and admissions
• Collaborate with intake, ED, and DPCS for care planning and education
• Provide patient/family education and support as part of the care plan
• Participate in QAPI activities as directed
• Maintain knowledge of hospice resources and guidelines
