Job summary

Bristol Hospice - Hawaii Llc is seeking a compassionate Registered Nurse to perform comprehensive patient and family assessments and deliver professional hospice care in the home/hospice setting.

You will develop and update the care plan with input from patients, families, and the interdisciplinary team; monitor pain and symptoms; educate patients and caregivers; and collaborate with colleagues to ensure high-quality end-of-life care. Hawaii license required.

Qualifications

• Must hold a current state RN licensure and be eligible to practice in Hawaii.
• Minimum of 1 year of acute care experience in an institutional setting; home care experience preferred.
• Flexibility in work hours and willingness to travel throughout Bristol Hospice Service Area.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families in hospice care.
• Develop and revise the plan of care with input from patient, family, and interdisciplinary team.
• Record pain and symptom management; communicate changes to physicians; provide patient and family education.
• Deliver professional nursing care following the nursing process and clinical competencies.
• Collaborate with the interdisciplinary group to ensure high-quality end-of-life care.
