The Colorado Coalition for the Homeless seeks a Registered Nurse (RN) for its Recuperative Care program. You will deliver high-quality nursing care to individuals recovering from illness or injury in a trauma-informed setting, collaborating with hospitals, clinics, and social service teams to ensure seamless care and recovery.

You will triage, educate, and support patients with chronic and acute conditions, while mentoring new nurses and maintaining precise EHR documentation to support holistic

Qualifications

Requires a nursing degree (BSN or MSN).
2+ years in ambulatory, public, community, mental health, emergency or hospice nursing preferred.
Experience serving diverse or marginalized communities, including homelessness, preferred.

Responsibilities

Provide nursing care to participants per accepted clinical standards.
Coordinate with hospitals, clinics, and social service agencies for continuity of care.
Triages and supports referrals based on acuity and bed availability.
Educate patients on chronic and acute conditions and health management.
Mentor nursing students and staff to enhance clinical skills.
Document all patient interactions accurately in the EHR.
