Job summary

Colorado Coalition for the Homeless is seeking an Registered Nurse to join the Recuperative Care program, delivering compassionate, trauma-informed healthcare to individuals experiencing homelessness. You will work in an integrated team with 24/7 patient access and a focus on safe recovery after illness or injury.

The role requires an RN license in Colorado, a nursing degree, and experience in relevant settings. Bilingual English/Spanish preferred.

Qualifications

• A degree in nursing from an accredited college or university is required.
• Colorado licensure as a Registered Nurse (RN) is required.
• 2 years minimum experience in ambulatory, public, community, mental health, emergency, hospice, or critical care nursing is preferred.
• Experience with serving diverse or marginalized communities, including homelessness, preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Provides nursing care through established standards of quality healthcare.
• Coordinates with hospitals, clinics, social services, and other CCH programs to ensure continuity of care.
• Triages and supports referrals based on patient acuity and bed availability.
• Performs rapid triage assessments in-person and by telephone with appropriate care plans.
• administers medications safely according to protocols.
• Educates patients on chronic and acute conditions and self-management strategies.
• Mentors and precepts new nurses and nursing students.
• Facilitates access to internal health outreach and housing support services.
• Documents all patient interactions in the EHR accurately and timely.
• Participates in team-based quality improvement initiatives.

Job Summary

The Registered Nurse (RN) in Recuperative Care plays a pivotal role in delivering high-quality, compassionate healthcare to individuals experiencing homelessness. This position requires clinical expertise within an integrated, trauma-informed care model, with a focus on patients recovering from illness or injury. The Recuperative Care program is a unique program supporting individuals who have been discharged from the hospital, who are too frail or complex to properly heal on the streets or in a shelter. Patients stay in the program and have access to clinical staff 24/7. The program comprises multiple disciplines working together to deliver holistic, integrated care.

Essential Job Functions

• Provides nursing care through accepted community standards of quality healthcare.
• Works with local hospitals, community clinics, social service agencies, and other CCH programs to ensure appropriate continuity of care.
• Triages and supports program referrals based on patient acuity, program requirements, and bed availability.
• Conducts rapid and accurate triage assessments both in-person and via telephone, providing appropriate care recommendations according to nursing protocols.
• Administers medications safely and effectively according to protocols.
• Educates and empowers patients by providing appropriate health education and information, addressing common chronic and acute health conditions such as diabetes management, hypertension management, nutrition, wound care, infectious disease, gender-affirming care, and family planning.
• Mentor and serve as a preceptor to new nurses and nursing students, enhancing their clinical skills and confidence.
• Facilitates access to internal programs such as health outreach and housing support services.
• Participates in team-based or interdisciplinary quality improvement initiatives.
• Attends required disciplinary, interdisciplinary, suite, quality improvement, and all-staff meetings.
• Ensures timely and accurate documentation of all patient interactions, clinical services, and communications in the electronic health record (EHR).
• Performs other duties as assigned.

Qualifications Summary
