Job summary

Virginia Garcia Memorial Health Center in Hillsboro, OR is seeking a Registered Nurse for the Recuperative Care Program. Based at VG7th Ave clinic with travel to the RCP site and local hospitals for discharge planning, under VG Medical Director supervision.

Qualifications

• Current Oregon RN license required.
• Experience with homelessness desirable.
• Epic experience preferred.

Responsibilities

• Screen referrals from hospitals and coordinate with discharge planners to assess patient acuity, program requirements, and bed availability.
• Develop care plans based on patient goals and discharge needs.
• Coordinate with shelter case management to address non-clinical needs and SDOH.
