Job summary

AdventHealth is seeking an RN to support readmission prevention and post-discharge care on a full-time basis in Ocala, FL. You will review CMS core measures, lead committee activities, and collaborate with care teams to improve patient outcomes.

Responsibilities include inpatient visits, patient education, and family communication, with frequent post-discharge follow-ups. A nursing license and EMR proficiency are required.

Qualifications

• RN with active license and 1+ year acute care experience.
• 2+ years acute care experience preferred.
• Proficient in Excel, Word, PowerPoint, and Teams.
• Experience with EMR systems and CMS care practices is a plus.

Responsibilities

• Review reports on core measures and readmissions daily.
• Coordinate Readmission Prevention Committee meetings and minutes.
• Participate in in-patient visits and post-discharge calls, using data to identify at-risk patients.
• Educate patients on disease management and medications; coordinate discharge needs.
• Collaborate with care teams to reduce readmission rates.

Job Responsibilities

• Reviews reports on Current Core Measure Hospitalizations and Re-admissions daily.
• Coordinates the Readmission Prevention Committee Meetings, identifies recent readmission cases, reviews for improvement opportunities, creates and sends out meeting agenda, and completes meeting minutes.
• Participates in in-patient visits and post-discharge phone calls, utilizing data reports to identify patients with Core Measure diagnoses.
• Visits patients in the hospital, provides education on disease process and medications, and collaborates with Care Management to address discharge needs.
• Collaborates with Respiratory Therapy and Cardiopulmonary Rehab Teams when appropriate.
• Contacts patients post-discharge, discusses patient health, provides education, reviews medications, and verifies follow-up appointments.
• Ensures post-discharge needs are arranged.
• Collaborates with multidisciplinary team to reduce readmission rates.
• Other duties as assigned.

• Experience in working with EMR [Required]
• Working knowledge of current care practices for the diagnostic groups identified by CMS [Preferred]
• Working knowledge of Federal and State regulatory requirements in hospital accreditation [Preferred]
