• Facilitates communication between nursing center departments and patient/family to support care plans.
• Maintains open lines of communication with interdisciplinary team.
• Coordinates and documents assessment/care plan meetings.
• Ensures care plans align with patient wishes and regulations.
• Monitors MDS process and educates staff on related procedures.
