Qualifications

• Nursing degree from an accredited program and GA RN license.
• Strong communication and organizational abilities.
• Proficient in Microsoft Office and patient data systems.

Responsibilities

• Facilitates communication between nursing and patient/family for care plans.
• Coordinates assessments, care plans and MDS-related activities.
• Leads nursing staff coordination and scheduling for the care team.
• Ensures compliance with regulatory requirements and quality standards.
