Qualifications

• Active RN license to practice in the State of Georgia.
• Nursing degree from an accredited college or university or graduate of an approved RN program.

Responsibilities

• Facilitates communication between nursing center departments and patient/family to provide an effective assessment/care plan.
• Establishes open lines of communication with interdisciplinary team to provide comprehensive care plan.
• Coordinates development of care plans (preliminary and comprehensive) with measurable goals.
