Job summary

Brookdale is seeking an RN supervisor to oversee resident assessments and care planning in a long-term care setting. The role requires coordinating MDS generation, ensuring compliance with state and federal guidelines, and leading a team through care planning and quality initiatives.

The position emphasizes training others in assessment methods and collaborating with discharge planning, ensuring timely documentation and ongoing program improvements across the community.

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Minimum two years of experience as a supervisor in a hospital, nursing care community, or other related health care facility.
• Minimum six months training experience in rehabilitative and restorative nursing practices.
• Knowledge of general, rehabilitative and restorative nursing and medical practices, procedures, regulations and guidelines governing long-term care.

Responsibilities

• Coordinate development and completion of resident assessments (MDS) in accordance with regulations and guidelines.
• Coordinate development of a written plan of care with measurable objectives and timetables.
• Ensure timely generation and transmission of MDS and maintenance of policies and procedures.
• Develop and maintain an ongoing quality assurance program for assessments and care plans.
• Assist the discharge planning process and participate in care plan meetings.
• Participate in surveys and committees related to care quality and compliance.
• Lead training sessions on assessment skills and MDS processes.
