Job summary

Healthmap Solutions is seeking a Quality Practice Advisor to coordinate performance improvement activities and engage providers to improve kidney health management across our member population.

You will partner with the Care Navigation team to enhance care coordination, educate on HEDIS measures, and ensure NCQA-compliant documentation and reporting. This role requires travel within the assigned region and a strong clinical operations background.

Qualifications

• Bachelor’s degree required.
• Active, unrestricted RN license required.
• 3 years of experience in a health care or managed care setting.
• 3+ years in healthcare services, clinical operations, quality, or care management.
• Proof of valid driver’s license and willingness to travel regionally.

Responsibilities

• Develop a trusting partnership with clinical providers and locations.
• Identify opportunities to improve health outcomes based on provider data.
• Educate on Best Practice sharing for low-performing areas.
• Assist in improving provider office workflows.
• Collaborate with physicians to identify members for Care Navigation support.
• Support stakeholders in development and execution of process improvements.
• Improve provider performance in Healthmap Solutions interventions and gaps.
• Educate providers on HEDIS measures and open care gaps.
• Support quality improvement interventions and NCQA compliance.
• Travel to practices to support programs as needed.
