Job summary

Healthmap Solutions seeks a Quality Practice Advisor to coordinate performance improvement activities and engage providers to enhance health outcomes. You will work with the Care Navigation team to strengthen the Kidney Health Management program and drive NCQA-compliant reporting across practices.

The role requires an RN with 3+ years in healthcare, BLS, and willingness to travel over 50% within an assigned geographic market in Florida. Bilingual Spanish/English is a plus.

Qualifications

• Bachelor’s degree required.
• Active, unrestricted RN license required.
• BLS certification required for field roles within 30 days of hire.
• 3+ years in healthcare or managed care.
• 3 years in claims or gap closure campaigns preferred.
• 3+ years in healthcare services, clinical operations, quality, or care management.

Responsibilities

• Develop partnerships with clinical providers and practice locations to improve outcomes.
• Identify opportunities to improve health outcomes using provider data.
• Educate on best practice sharing to address low performance areas.
• Assist in identifying areas for process improvement in provider workflows.
• Collaborate with Care Navigation to identify members benefiting from support.
• Support stakeholders in planning and executing quality improvement initiatives.
• Improve performance in interventions, care gaps, and chronic conditions.
• Educate providers on HEDIS measures and open Care Gaps.
• Maintain documentation per NCQA standards and deliver timely reports.
• Travel within assigned region to support practices.
