Healthmap Solutions is seeking a RN Clinical Liaison/Quality Practice Advisor to coordinate performance improvement activities and engage Care Navigation teams to enhance health outcomes for members. The role supports education, provider engagement, and NCQA-aligned documentation across the Kidney Health Management program.

The position requires an RN license, Bachelor's degree, BLS certification within 30 days, and 3+ years in healthcare/managed care, with substantial travel (over 50%) within

Qualifications

• Bachelor’s degree required.
• Active, unrestricted RN license required.
• BLS certification required within 30 days of hire.
• 3 years of experience in a health care or managed care setting.
• 3+ years in healthcare services, clinical operations, quality, or care management.
• Proven ability to build and manage relationships with health care providers.
• Driver’s license and automobile insurance; regular travel within assigned region.

Responsibilities

• Develop partnerships with clinical providers and practice locations.
• Identify opportunities to improve health outcomes using provider data.
• Educate on Best Practice sharing for underperforming areas.
• Support provider office process improvement initiatives.
• Collaborate with Care Navigation to enhance care coordination.
• Maintain documentation per NCQA standards.
• Deliver timely reports to stakeholders.
• Perform other related duties as assigned.
