Job summary

Healthmap Solutions seeks an RN Clinical Liaison/Quality Practice Advisor to coordinate performance improvement activities and engage providers to enhance health outcomes for Healthmap Solutions members. You will partner with Care Navigation to strengthen the Kidney Health Management program.

Location options include Springfield, MA or Hartford, CT with heavy travel (>50%). The role requires RN licensure, BLS, and NCQA-aligned quality improvement experience to drive provider engagement and

Qualifications

• Bachelor’s degree required.
• Active, unrestricted RN license required.
• BLS certification required within 30 days of hire.
• 3 years of healthcare or managed care experience.
• 3+ years in healthcare services, clinical operations, quality, or care management.
• Valid driver’s license and willingness to travel regularly.

Responsibilities

• Develop partnerships with clinical providers and sites to improve outcomes.
• Identify opportunities to improve health outcomes using provider data.
• Educate on best practices and close care gaps in NCQA-aligned workflows.
• Support process improvement in provider office workflows.
• Collaborate with Care Navigation to enhance care coordination for Healthmap Solutions members.
• Document meetings and interactions for coordination and compliance.
