Job summary

Nevada Health Centers is seeking a Quality & Population Health Manager who is an experienced registered nurse responsible for leading quality improvement across patient safety and population health initiatives.

Qualifications

• Must be a graduate of an accredited nursing school.
• Nevada RN license required.
• Knowledge of FQHC operations, HRSA requirements, NCQA standards, and UDS reporting.
• Current CPR or BLS certification.
• Minimum of five years in quality improvement and case management.
• Experience with electronic health records.

Responsibilities

• Supports and supervises RN Case Managers.
• Coordinate monthly Quality Improvement and QA Committee meetings.
• Collaborate on quality initiatives focusing on patient outcomes, safety, experience, and performance.
• Participate in risk assessment and mitigation initiatives.
• Monitor, analyze, and report on quality metrics.
• Oversee population health programs and chronic disease initiatives.
• Collaborate with health plan quality teams for care transitions.
• Organize staff meetings, huddles and rounding.
• Assist in budgeting and HR for the department.
• Coordinate services and evaluation of care for high-risk patients.
• Develop policies and staff education for quality and population health.
• Work with population health data analysis to share actionable data.
• Identify KPIs with Director of Population Health.
• Coordinate grants and programs.
• Lead accreditation efforts, e.g., PCMH.
• Conduct quarterly quality chart reviews.
• Participate in health plan quality meetings.
• Coordinate quarterly on-site quality rounds.
