Job Responsibilities

• Coordinate efforts to obtain and maintain certifications of excellence including Heart Failure and Stroke.
• Coordinate Quality Improvement (QI) efforts related to data collection and reporting for Disease Specific Certifications/Accreditations.
• Manage activities and metrics related to disease-specific care for HF and Stroke.
• Collect, analyze, and present QI data including Core measures, hospital quality data, complaint data, and peer review data.
• Ensure hospital-wide compliance with certification standards.
• Lead performance improvement teams for disease-specific care.
• Provide staff education and serve as a resource for hospital staff and leaders.
• Lead accreditation survey activities.
• Provide new employee orientation on quality standards.
• Participate in interdisciplinary team rounds.
• Implement problem-solving processes to enhance patient care.
• Lead community outreach efforts related to disease-specific care.
