Job summary

The RN Program Coordinator at ProMedica is the point person for ensuring collaborative care of patients from hospital discharge through all follow-ups. In their coordination, RN Program Coordinators facilitate the patient plan of care and provide education for procedures and testing ordered by providers.

Qualifications

• Current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification.

Responsibilities

• Coordinate patient care from discharge through follow-ups and facilitate the plan of care.
• Provide education for procedures and testing ordered by providers.
• Review Health Risk Assessments and monitor test results with providers.
