Job summary

ProMedica Central Physicians seeks an RN Program Coordinator to ensure collaborative care from discharge through follow-ups. You will coordinate patient plans of care, educate patients on procedures, and review Health Risk Assessments with providers and patients.

Responsibilities include monitoring charts for test results, managing preoperative clearances, and triaging calls with health advice or referrals as appropriate. This role supports perioperative coordination in obstetrics/gynecology.

Qualifications

• Licensed RN in state of Ohio.
• ACLS and BLS certification.
• 1 year of relatable experience.
• BSN preferred.
• EPIC experience beneficial.
• 2+ years relatable experience preferred.

In their overall coordination, RN ProgramCoordinators facilitate the patient plan of care, provide education and instructions for procedures and/or testing ordered by providers, and review the patient’s Health Risk Assessment.The RN Program Coordinator monitors patients’ charts for test results and reviews them with the provider and the patient. You will also review and manage preoperative surgical clearance requests with providers, if needed.In addition to these responsibilities, you may also triage patient calls and provide health advice and referrals as appropriate.The above summary is intended to describe the general nature and level of work performed in this role.
