Job summary

ProMedica Health and Wellness Center in Ohio is seeking an RN Acute Care Navigator to oversee patient care, ensure appropriate care progression, and coordinate discharge planning within the Pulmonary department.

You will collaborate with interdisciplinary teams, perform initial assessments, and educate patients on medications, treatments, and post-discharge care to optimize outcomes and reduce readmissions.

Qualifications

• Must have BSN or 5 years' applicable nursing experience.
• Current, active, and unrestricted RN licensure in the state of practice is required.
• MSN or higher is preferred.
• Experience in patient advocacy, care management, and knowledge of hospital/community resources is preferred.
• Case Management or similar experience is a plus.

Responsibilities

• Oversee patient care and ensure appropriate level of care for caseload.
• Collaborate with interdisciplinary teams to conduct assessments and develop discharge plans.
• Educate patients on medications, treatments, and post-discharge care.
• Monitor patient progress and review health records to escalate issues for smooth transitions.
