Job summary

ProMedica is seeking an RN Program Coordinator in Toledo for part-time days in the Cardiology department. The role focuses on ensuring seamless patient care from discharge through follow-up, coordinating plans of care, educating patients on procedures, and reviewing health assessments with providers.

The coordinator will monitor charts for test results, manage preoperative clearances, and may triage calls with health advice or referrals as appropriate.

Qualifications

• Current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification.

Responsibilities

• Coordinate patient plan of care from discharge through follow-ups.
• Provide education and instructions for procedures/testing ordered by providers.
• Review Health Risk Assessments with patients and providers.
• Monitor patient charts for test results and discuss findings with care team.
• Review and manage preoperative surgical clearance requests with providers.
• Triage patient calls and provide health advice and referrals as appropriate.
