Crossover Health is seeking a Registered Nurse to join its primary care team. The RN will provide direct patient care, education, and participate in an interdisciplinary care model focused on wellbeing and prevention.

Responsibilities include care coordination, triage, phlebotomy, and vaccination administration in a collaborative clinic environment. The ideal candidate has at least 3 years of clinical experience, an active RN license, and BLS certification, with a BSN preferred.

Qualifications

• Graduate from an accredited school of nursing.
• Minimum 3 years of clinical experience.
• Current Registered Nurse license in the state you are practicing.
• Current BLS (Basic Life Support) certification.
• BSN (preferred).
• Outpatient care and urgent care experience.
• Knowledge of health tech and software commonly used in clinical settings.
• Commitment to delivering patient-centered, high-quality care.

Responsabilités

• Performs evidence-based nursing care and patient education.
• Care management by connecting patients with specialists and following up.
• Uses population health tools to ensure optimal care as per guidelines.
• Provides phlebotomy services and in-house lab work with providers.
• Administers vaccines and travel-related immunizations.
• Manages RN schedule and triage responsibilities.
• Complies with policies and standing orders; supports quality improvement.
• Participates in training and onboarding of new nurses.
• Assists in reporting incidents and compliance activities.
