Job summary

Crossover Health is seeking an experienced Registered Nurse to join our on-site care team. The RN will provide direct patient care, education, and coordination across disciplines to support a proactive, wellbeing-focused model.

The role requires a valid RN license in Texas, a minimum of 3 years of clinical experience, and BLS certification. You will work within an interdisciplinary team and participate in population health initiatives and quality programs.

Qualifications

• Graduate from an accredited school of nursing.
• Minimum 3 years of clinical experience.
• Current Registered Nurse license in the state of practice.
• Current BLS certification.

Responsibilities

• Provide evidence-based nursing care and patient education.
• Perform care management by connecting patients with specialists and following up.
• Utilize population management tools to ensure optimal care.
• Provide phlebotomy, in-house labs, and coordinate with providers for tests.
• Administer vaccines and travel immunizations as needed.
• Manage RN schedule and triage needs for appointments and walk-ins.
• Contribute to quality improvement programs and incident reporting.
