Job summary

Suncoast Hospice, part of Empath Health, seeks a compassionate RN Case Manager to cover Pinellas County, delivering high-quality end-of-life care in home and facility settings. You will review care plans, educate patients and families, coordinate physician orders, and work with a team to ensure comfort, dignity, and safety for clients with advanced illness.

A Florida RN license, nursing degree, and at least one year in acute care are required; hospice experience is preferred.

Qualifications

• Graduate of an approved nursing program.
• Current Florida RN license.
• Minimum one year experience in an acute-care hospital environment providing direct patient care; hospice RN preferred.

• Educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.

This is not a requirement if the colleague chooses an alternative means of transportation when reassigned to another facility based on business needs. Regardless of the mode chosen, both are at the colleague’s own expense, with the exception of mileage reimbursement eligibility when traveling between facilities while on company time.
