Job summary

Suncoast Hospice, part of Empath Health, seeks an RN Case Manager in Palm Harbor, FL. You will coordinate patient and family care, perform comprehensive assessments, and guide the care team through the hospice journey with compassion.

Responsibilities include developing the Plan of Care, supervising LPNs and aides, and ensuring timely medication administration and education to patients and families. Daytime hours, Monday–Friday, in North Pinellas County.

Qualifications

• A.A. degree or equivalent from two-year college or technical school.
• Minimum of one year related experience and/or training.
• Valid Florida license as a Registered Nurse and valid Florida State Driver’s License.
• Bilingual Spanish/English preferred.
• For field-based positions, reliable transportation and proof of automobile insurance.

Responsibilities

• Obtain patient history, perform age-specific physical, psychosocial and spiritual assessments and initiate appropriate referrals and collaboration with IDT and facilities.
• Develop and update the Plan of Care with patient, family, IDT, attending and physician.
• Provide continuity of care, care coordination and evaluate progress toward goals.
• Assist with closure contact and support after a patient’s death, in collaboration with IDT.
• Direct and supervise care provided by LPNs and Home Health Aides and evaluate services every two weeks.

Monday - Friday, daytime hours. Coverage area includes North Pinellas County, providing direct patient care in facilities. As an RN Case Manager, you'll build meaningful relationships with patients and families, guiding them through their hospice journey with compassion, expertise, and dignity.Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way.## **What you'll Do*** Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.* As RN Case Manager provides continuity of care, care coordination, identifies patient/family goals, develops a Plan of Care with patient, family, IDT, attending and physician, partnering facility and evaluates progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change.* Following a patient’s death, assists with closure contact and support in collaboration with IDT.* Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.* Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician.
