UCHealth Primary Care Mason Street in Fort Collins, CO seeks a registered nurse to provide direct patient care in a physician office setting. You will assess, plan, implement, and evaluate care using the nursing process while aligning with UCHealth policies and mission.

Responsibilities include coordinating care across disciplines, teaching patients and families, and supporting a fast-paced clinical environment. RN licensure and BLS are required; Bachelor’s degree in Nursing preferred.

Qualifikationen

• Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years experience.
• Preferred: Bachelor’s degree in Nursing.

• Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize resources.
• Monitors, records, and communicates patient condition; coordinates care across disciplines, including transfer, discharge, referral, and psychosocial needs.
• Provides patient/family education appropriate to age, culture, condition and circumstances.
• Delegates patient care to ancillary staff and may precept students or new hires.
• Demonstrates critical thinking, decisive judgment, and ability to work with minimal supervision in a fast-paced environment.
