Concierge Home Care in West Palm Beach, FL is seeking an experienced Home Health RN Preceptor to mentor clinicians, lead onboarding, and support clinical excellence with a manageable patient caseload.

This leadership role offers flexible scheduling, autonomy over daily workflow, and regional travel across Sarasota County and surrounding areas while maintaining a strong focus on quality patient care.

Qualifications

Active Florida RN license required.
Minimum 2 years of home health experience.
Strong knowledge of Medicare OASIS documentation.
Experience with WellSky/Kinnser EMR preferred.

Responsibilities

Conduct co-visits to train and support field clinicians.
Lead clinical onboarding and orientation sessions.
Complete competency evaluations and skill validations.
Provide ongoing mentorship and clinical guidance.
Perform patient assessments, OASIS documentation, and care planning.
Deliver skilled nursing care including chronic and acute disease management.

RN Preceptor Responsibilities

Conduct co-visits to train and support field clinicians
Lead clinical onboarding and orientation sessions
Complete competency evaluations and skill validations
Provide ongoing mentorship and clinical guidance
Perform patient assessments, OASIS documentation, and care planning
Deliver skilled nursing care, including:
- Chronic and acute disease management
- Medication management
- Wound care and post-surgical care
- Pain management
Collaborate with physicians and interdisciplinary teams
Support quality assurance and compliance initiatives
Travel within Sarasota County and surrounding areas as needed

Schedule & Compensation
