Concierge Home Care in West Palm Beach, FL seeks an experienced Home Health RN Preceptor to mentor clinicians, manage a reduced patient caseload, and lead onboarding while maintaining a balanced schedule.

The role offers autonomy, regional travel across branches, and $85K+ base with quarterly bonuses. Eligible benefits include health/dental/vision insurance after 30 days and PTO, with EAP support.

Qualifications

• Active, unencumbered Florida RN license (required).
• Minimum 2 years of home health experience (required).
• Strong knowledge of Medicare OASIS documentation (required).
• Experience with WellSky/Kinnser EMR (preferred).

Responsibilities

• Conduct co-visits to train and support field clinicians.
• Lead clinical onboarding and orientation sessions.
• Complete competency evaluations and skill validations.
• Provide ongoing mentorship and clinical guidance.
• Perform patient assessments, OASIS documentation, and care planning.
• Deliver skilled nursing care including chronic and acute disease management.
• Collaborate with physicians and interdisciplinary teams.
• Travel within Sarasota County and surrounding areas as needed.
