VNS Health Home Care RNs deliver patient-centered care in New York communities, helping patients heal at home and avoid unnecessary hospital visits. You will join a robust interdisciplinary team, providing personalized nursing and coordinated care management to diverse patients in homes and facilities.

The role emphasizes independent practice, ongoing assessment, and timely plan-of-care adjustments while adhering to high standards of clinical quality and safety within our 130-year-old mission.

Qualifications

• NY State RN license and registration required.
• Valid driver’s license required.
• OASIS certification preferred.
• Hospice-specific certifications may apply.

What You Will Do

• Practice independently in the community as part of an interdisciplinary care team.
• Deliver personalized nursing and care management to patients in their home or care facilities.
• Constantly evaluate evolving patient needs and respond with plan of care adjustments.

Qualifications

Licenses and Certifications: A license and current registration to practice as a Registered Professional Nurse in New York State required and Valid driver’s license required and OASIS certification preferred

For Hospice Only:

Wound Care Certification required or willingness to obtain within one (1) year of hire required

Certification in Hospice and Palliative Care Nursing required

Education: Bachelor's Degree in Nursing OR Associate’s degree in Nursing required

Work Experience: Four years’ clinical home care experience required and Minimum of two years full-time clinical home care experience required and Minimum of one year of experience as a field staff nurse in VNS Health Home Care, which includes ability to manage/document cases with a minimum of supervision, identify/assess/formulate/implement action plan to address problem and evaluate outcome preferred and Joint visits with other nursing staff to demonstrate new techniques preferred Hospice Only: four years' clinical hospice experience or clinical home care experience Hospice Only: Minimum of two years' full-time clinical hospice or home care experience

Pay Range

USD $126,413.00 - USD $160,188.00 /Yr.
