Job summary

VNS Health in New York seeks a qualified registered nurse to provide personalized home-based care as part of an interdisciplinary team. You will develop and manage individualized plans of care, assess changing patient needs, educate patients and caregivers, and use AI-powered documentation to stay focused on delivering high-quality care.

We offer 4 weeks of paid clinical orientation, 31 days of paid time off plus 9 holidays, tuition reimbursement, CEU credits, and ongoing career development

Qualifications

What You'll Do

• Provide personalized nursing care in patients' homes as part of an interdisciplinary care team
• Develop and manage individualized plans of care

• Use available clinical technology, including AI-powered documentation support, to support efficient and accurate

Licenses And Certifications

• License and current registration to practice as a registered professional nurse in New York State required.
• Valid driver's license or NYS Non-Driver photo ID card may be required as determined by operational/regional needs.
• Wound Care Certification required, or willingness to obtain within one year of hire preferred.
• Certification in Hospice and Palliative Care Nursing preferred.

Education

• Degree in Nursing from an approved program required.

Work Experience

• Minimum of one year of clinical nursing experience, including at least six months of Hospice, Med/Surg, ICU, Emergency Department, Home Care, Palliative Care, Oncology or Long Term Care experience required.
• Demonstrated basic computer skills required.

Pay Range

USD $118,942.00 - USD $152,717.00 /Yr.
