Job summary

VNS Health seeks a registered nurse to deliver personalized in-home care in New York. You will work with an interdisciplinary team to develop and manage care plans, assess patient needs, and educate patients and caregivers throughout the care journey.

What You'll Do

• Provide personalized nursing care in patients' homes as part of an interdisciplinary care team

Qualifications

What You'll Need

Licenses and Certifications:

• License and current registration to practice as a registered professional nurse in New York State required.
• Valid driver's license or NYS Non-Driver photo ID card may be required as determined by operational/regional needs.
• Wound Care Certification required, or willingness to obtain within one year of hire preferred.
• Certification in Hospice and Palliative Care Nursing preferred.

Education:

• Degree in Nursing from an approved program required.

Work Experience:

• Minimum of one year of clinical nursing experience, including at least six months of Hospice, Med/Surg, ICU, Emergency Department, Home Care, Palliative Care, Oncology or Long Term Care experience required.
• Demonstrated basic computer skills required.

USD $118,942.00 - USD $152,717.00 /Yr.
