Job summary

Crystal Creek Post-Acute in Stockton, CA is seeking a Registered Nurse (RN) to supervise nursing staff and provide direct resident care in a skilled nursing environment.

Responsibilities include assessments, care planning, medication administration, progress monitoring, and collaborating with interdisciplinary teams to deliver high-quality care. California RN license and BLS are required; new graduates are welcome to apply.

Qualifications

• Associate’s or Bachelor’s degree in Nursing required.
• Current California RN license and active BLS certification required.
• Newly licensed RNs are encouraged to apply.

Responsibilities

• Supervise nursing staff and provide total nursing care for residents.
• Conduct thorough assessments and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor residents’ progress and adjust care plans as needed.
• Educate residents and families about health conditions and care options.
• Collaborate with interdisciplinary teams to ensure optimal care.
• Document all patient care accurately in electronic records.
