Canyon Creek Post-Acute in Castro Valley, CA is seeking a Registered Nurse (RN) for PRN/on-call coverage across AM, PM, and NOC shifts. This on-site role focuses on supervising nursing staff and delivering total resident care.

The ideal candidate holds an Associate's or Bachelor's in Nursing, a California RN license, and BLS/CPR certification, with prior SNF experience preferred. We offer comprehensive benefits and a supportive team.

Qualifications

• Associates or Bachelor’s degree in Nursing.
• Registered Nurse (RN) license in California.
• Six months of experience in a skilled nursing environment is desirable.
• Active BLS/CPR.

Responsibilities

• Supervises nursing staff and coordinates nursing care for residents.
• Ensures timely physician interventions and effective communication.
• Schedules nursing unit staff and directs care per state regulations.
• Monitors meals and 10-minute breaks to ensure compliance.
• Coordinates use of personnel, supplies, equipment, and facilities.
• Participates in nurse on-call system and adjusts staffing as needed.
• Obtains medications, supplies, and medical records for safe care.
• Completes discharge capitulation summaries within five days.
