Job summary

The Redwoods Post-Acute Team in Willow Glen, San Jose, seeks a dedicated Registered Nurse to join our 152-bed skilled nursing facility. You will assess residents, develop care plans, administer medications, and collaborate with interdisciplinary teams to ensure high-quality, patient-centered care.

Qualifications

• Fresh or newly licensed RNs are welcome.
• Experienced RN is preferred.
• Experience in a skilled nursing facility/long-term care/senior care is a plus.
• A valid and active California license as a Registered Nurse is required.
• Must have an active BLS through the American Heart Association.
• Must have an active CPR Certification.
• Interest in working with the geriatric population.
• Ability to work in a fast-paced environment and manage multiple priorities.
• Willingness to work flexible hours, including weekends and holidays as needed.

Responsibilities

• Conduct thorough assessments of residents' health status and needs.
• Develop and implement individualized care plans based on assessments.
• Administer medications and treatments as prescribed by physicians.
• Monitor residents' progress and make necessary adjustments to care plans.
• Provide education to residents and families about health conditions and care options.
• Collaborate effectively with interdisciplinary teams to ensure optimal care.
• Document all aspects of patient care accurately and timely in electronic records.
