Job summary

Redcrest Home Health seeks a Post-Acute Care Coordinator (PACC) to manage discharge planning and coordinate home-based care for patients at IU Methodist Hospital. You will attend rounds and support care transitions across settings.

Role requires an active RN license, at least two years of experience (home care preferred), and a valid driver's license. We offer a $10,000 sign-on bonus and a comprehensive benefits package.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum of two years of experience. Home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Determine home care eligibility and review patient insurances and medical documentation.
• Coordinate health care services as ordered by the attending physician.
• Assist hospital/facility personnel in the discharge planning process.
• Ensure coordination of all ancillary services per the patients’ needs following discharge.
