Job summary

CHI Health Schuyler is seeking an Population Health Coach RN to lead care coordination across clinic, post‑acute, and inpatient settings in Nebraska. You will use the Innovaccer platform to monitor high‑risk patients, close gaps, and ensure seamless transitions with physicians and staff.

You will coordinate referrals for social work, dieticians, and prescription assistance while educating patients on new treatments and preventive care. Strong organizational skills are essential.

Qualifications

• Nebraska RN license required (Compact accepted)
• Associate Degree in Nursing is required; Bachelor’s preferred
• 2+ years nursing experience required; 3–5 years preferred
• Experience with care coordination and population health programs a plus

Responsibilities

• Care Management & Outreach: manage high‑risk patients and close care gaps, including wellness visits and screenings.
• Seamless Care Transitions: coordinate ED/inpatient discharge follow‑ups for home care.
• Interdisciplinary Care Coordination: connect social work, dieticians, and pharmacy assistance.
• Evidence-Based Practice Implementation: apply metric guidelines to improve care quality and patient experience.
• Clinical Performance Tracking: use ICT platforms to monitor workflows and report outcomes.
