Franciscan Healthcare is seeking a Health Coach RN to join our team in West Point, NE. The role focuses on nursing care, case management, care transitions, diabetes and chronic disease education, and population health initiatives to empower patients in making informed health decisions.

The RN will coordinate assessment, care planning, and evaluation across the lifespan, emphasizing patient advocacy and collaboration with health professionals. Hours: 35+ per week, Mon–Fri with rotating Saturdays.

Qualifikationen

• Graduate of an accredited school of nursing with a current licensure as a Registered Nurse in the State of Nebraska.
• Minimum 1 year experience in home health, med/surg or critical care area preferred.
• Basic Life Support or ability to obtain within probationary period.
• Interpersonal skills requiring courtesy and tact are necessary to communicate effectively.

• Prioritizes, plans, and provides direct care to assigned patients during the shift.
• Relays pertinent information through appropriate documentation and referral to other health team members.
• Initiates and implements patient teaching plan, evaluates it, and revises as needed to meet learning needs of patients and families.
• Supports an environment conducive to professional practice by mentoring and serving as a resource person.
• Provides cost effective, quality patient care through appropriate use of supplies and personnel.
• Acts as a patient advocate with physicians and other health care providers.
