Job summary

Qualifications

Responsibilities

• Provides education and preventive health measures for patients with chronic disease to set behavior change goals.
• Works to control costs while ensuring appropriate staffing, equipment, supplies, and patient care.
• Adheres to restorative care philosophy in daily direct care of residents.
• Assesses, develops, and implements clinical program components to optimize patient care strategies.
• Demonstrates interviewing and assessment skills to plan, implement, and evaluate patient care with residents and families.
• Delivers timely, professional customer service to internal and external stakeholders.
