Job summary

Cleveland Clinic Health System is seeking an RN Care Coordinator for Pediatric Cardiology at the Main Campus in Cleveland. You will coordinate care for high‑risk pediatric patients, working with a multidisciplinary team to develop plans, manage disease care, and ensure smooth transitions across settings.

Ideal candidates will have an RN licensure, BLS certification, 3–5 years of nursing experience, and a willingness to pursue cancer-specific certification if applicable.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) Certification through the American Heart Association or American Red Cross.
• Three to five years of nursing experience.
• Any RN or APRN must obtain a cancer-specific certification or ongoing qualifying education within the accreditation cycle when working in cancer-related units.

Responsibilities

• Collaborate with a multidisciplinary care team to develop goals and interventions for high-risk patients.
• Provide care and disease management coordination across the continuum.
• Identify patients needing ongoing coordination and conduct targeted outreach.
• Perform comprehensive clinical assessments including disease, age, medical, behavioral, and social needs.
• Educate patients and families, monitor adherence, and update care plans as needed.
• Act as liaison and advocate for patients and families across transitions of care.
