Job summary

CHOC in Orange, CA, seeks an experienced pediatric care coordination nurse to deliver family-centered care in the clinic. You will collaborate with nurses, social workers, physicians and caregivers to plan and implement care across the continuum, advocating for optimal health and timely access to services.

Must have BSN in nursing; MSN preferred. BLS and CA RN license required; Case Management certification preferred. CCS experience desirable; strong communication and teamwork essential.

Qualifications

• Bachelor degree in nursing is required.
• Master's degree in nursing is preferred.
• Certification in Case Management is preferred.

Responsibilities

• Delivery of care coordination services for patients in the clinic setting.
• Planning and delivery of family-centered patient care through collaboration with patient, family, nurses, social workers, physicians and other practitioners.
• Advocating for optimal health, access to care, and appropriate resources.
• Collaborating with payers and interdisciplinary teams to ensure quality patient outcomes.
• CCS paneling to provide collaborative comprehensive care in a multidisciplinary setting.
