Job summary

Responsibilities

Conduct comprehensive assessments across physiological, psychological, development, sociocultural, spiritual and lifestyle factors.
Develop and implement individualized care plans and revise as needed.
Administer medications and treatments per orders and protocols; monitor for adverse reactions.
Educate patients and families about conditions, treatments and self-care strategies.
Collaborate with medical staff to coordinate patient care and ensure effective communication.
Document patient information, care activities and outcomes accurately.
