Job summary

SSM Health is seeking an RN to join the Transfer Center and coordinate patient transfers from external facilities, physician offices and clinics. The role emphasizes timely communication, multidisciplinary collaboration, and adherence to EMTALA and transfer protocols.

Ideal candidates have 5+ years of RN experience in acute care and possess RN licensure in Illinois or other applicable states. The position works night shifts with a 36-hour weekly schedule, and a sign-on bonus may be available.

Qualifications

• Five years registered nurse experience in acute care hospital setting.
• Graduate of accredited school of nursing or education equivalency for licensing.
• RN license in Illinois or compact/multi-state licenses (Illinois IDFPR; Missouri/Oklahoma/Wisconsin compact provisions) with potential for Illinois IDFPR within 6 months.

Responsibilities

• Facilitates, tracks and documents hospital transfers in a customer-centric Transfer Center.
• Responds quickly to calls, manages the call queue with strong customer service.
• Collaborates with facilities to obtain medical and administrative approvals.
• Adheres to EMTALA and program requirements; communicates ETA to receiving teams.
• Maintains accurate database of transferred patients from referral to admission and placement.
