Job summary

Community Health Systems is looking for a Patient Navigator - RN for a full-time position in the Cath Lab at Northwest Medical Center in Tucson, Arizona. In this role, you will provide guidance and support to patients and their families throughout the continuum of care and assist in coordinating care and overcoming various barriers.

The ideal candidate will have significant experience in clinical nursing and a strong background in patient navigation or care coordination. Additional qualifications include Licensure as an RN and Basic Life Support certification.

Qualifications

• 3-5 years of clinical nursing experience required.
• 1-3 years of experience in patient navigation or care coordination preferred.

Responsibilities

• Serves as the primary point of contact for patients from diagnosis through treatment.
• Assesses patients’ medical, emotional, and socioeconomic needs.
• Facilitates communication between patients, caregivers, and healthcare providers.
• Coordinates referrals to support services such as social workers and dietitians.
